Not every playthrough, but probably most. I usually try something completely opposite of what I did last, and just use it as a quick refresher of the mechanics etc. Specially if I plan on playing a melee build.

I played it twice. Once on my first run through because I needed to learn the game.
Once on my 2nd run through because the game was bugged and saying no resulted in you having to do it regardless.
