He was very very rich in a world with advanced medical technology for those who can afford it.

You’re lucky to make it past 30 if you’re a nobody because you’ll get shot in the street or die from malnutrition.
Saburo was the richest, most powerful man in the World for well over a century. A world where body augmentation through cybernetic implants is rampant.
The whole reason he invented the Relic was to cheat death when technology could no longer sustain his body. It’s largely implied that Yorinobu stole the Relic because he knew Saburo was going to use it to body snatch him.
Cheating death has been Saburo’s only goal since he tried to commit seppuku after World War Two.

Poor people quite often don’t make it past 30 in NC. Really rich people can look and behave 30 when they are 89 (i.e. Kerry Eurodyne). Cyberpunk is a warning about our development towards an extremely unjust society.
Saburo definitely cares about staying alive and has used all sorts of methods to live until 150 before he started looking at the Relic to keep him going forever in new bodies.

According to the wiki, very early Cybernetics started to appear in the early to mid 1990s. So if Saburo who was born in 1919 would be in his early 20s by the time the US and Japan were fighting in World War II. He would have been in his 70s by the 1990s and I guess by then he would have used those early cybernetics.
I would assume that least some prototypes existed during the 1980s or even earlier and that perhaps Saburo was among the very first humans to start getting cybered up as way to extend his lifespan.

I believe canonically he expected to die in a few years, part of why he had the Mikoshi engram plan going, man could NOT let go.

Remember that Rogue and Kerry are in their 80s, if you got the Eddies, you can live a real long time and look good doing it. If Relic actually takes off in the future it could make you immortal via body hopping a la altered carbon

Isn’t there a voice line from Jackie during the raid that says he’s at least 140? Also I think most people are forgetting that Saburo doesn’t actually live in NC, the average life expectancy there might be low thanks to corpo nonsense and gang violence so frequent that NCPD regularly outsources their work to mercs, but afaik that’s not necessarily the case for everywhere else in the world, especially since NC is actually technically independent, I would imagine most other places have some level of government regulation that would prevent the level of anarchy present in NC.

Correction, your lucky to make it past 30 if you are poor in Night City (or anywhere else in the world),
That was not a problem for the CEO/leader of Arasaka.

You dont have to worry about that when you can replace 99% of your body

Likely that the only person with more machine parts in his body than Saburo is Smasher.
gothic_they
a combination of lots and lots of wealth, bacta-tank like regeneration (submerged in a liquid completely) and in my opinion, purely out of spite of militech and yorinobu. Cybernetics would have played heavily into this as well
Makes me think how long someone like Adam Smasher (full-body conversion) or lizzy wizzy where pretty much the brain is the only biological part left.

Nanotech can extend your lifespan by a LOT in Cyberpunk. Hanako is also in her 80s, if memory serves.

This is actually the prediction with the highest likelihood of coming true. Right people will get nanobots, or whatever it takes to stay alive.

Drugs, implants and implanted drugs.
Same goes for Yorinobu and Hanako, they are 82 and 78 years old respectively yet they both look in their late 30s at worst. It’s clear from TTRPG books to the game scenario, Arasaka’s grand plan is cheating death and Relic is an extension of that plan.
