Yes, unfortunately! If the symbols aren’t showing at the foot of the door, like where a doormat would go, then the symbols are wrong. I guess you could also go through one of the other doors and look to make sure the floating boulder is right above the altar in the Aether Nexus.
Yep, if you get them correct the symbols will also be on the floor of the portal before going through. If they are not there, don’t go through.

Yeah if you get the code wrong you fall to your death. Stamina-Up with the Free Faller Augment negates the fall damage in case you get it wrong
Use the stamina up augment to not take fall damage.
