I don't know whether it was a creative choice, or whether they are hyper rare, but it seems to me that this game really hates generating tier five variance of almost every weapon modification. Only two or three attachments get the treatment, and everything else (i.e., the items that I actually want), never go above tier 4. And while I could put a lower level mod onto my iron, I hate the idea of having level 4 attachments on level 5 iconic weapons. Especially when it comes to the non-removable internal mods.

Just make the legendary mods, you can combine 2 of the lower tier to get the next. That’s how I got mine.
For the non-removable mods, you can craft a higher tier by combining two lower level ones. By buying up the stocks at all weapon vendors, this is pretty easy to get tier 5 non removable mods, as early as level 30.
For the scope and muzzle, I feel you. The only work-around I found to consistently get tier 5 scope and muzzle is to do the driving mission for El Capitan (in the PL DLC). His rewards tend to have one or two tier 5 scope and muzzle.
You can at least use lower tier mods to craft higher tier ones. Why the fuck we cannot buy, craft or find high tier attachments is beyond me.
