So, I know some characters, primarily Rogue, have asked V "what is Johnny saying?" But I don't reall anyone saying that they can actually hear V talking to Johnny.
What do you think, my choombas, can people hear V talk to Johnny?
I always thought it was more on V’s mind since we see a moment, during the first reveal in-game that Johnny is a thing, where they mirror each other’s movements.
As you said, i can’t really recall any moment where we talk to Johnny and someone asks us “who the heck are you talking to?”

it is literally shown in the diner scene after takemura leaves, V speaks to johnny as if he was there and johnny tells V that V doesnt have to do that as he is just in Vs head

Except for their very first interaction in the diner (and maybe also in V’s apartment beforehand) V simply talks to Johnny by projecting their thoughts in their head.
Only cases for exception are Songbird, due to her connection to the relic, and Judy briefly hearing Johnny during their joint-BD scrolling where they could hear each other’s minds.

In their first few scenes together, V speaks aloud when talking to Johnny, and of course other people can hear that. Johnny points out in Tom’s Diner after the meeting with Takemura that V doesn’t have to do that, and from then on, your conversations with him are totally in your (shared) head–the game actually adds a faint reverb effect to V’s voice during these conversations to show that it’s V “thinking” rather than speaking.
Rogue can’t see or hear Johnny, but she knows Johnny well and her guesses about when he’s butting into the conversation and what he’s saying are usually pretty spot-on. Kerry gets a couple of these too.
Songbird can see and hear Johnny, because she’s talking to you through the chip the same as he is. She’s the only other character who can sense him, outside of the faint “echoes” Judy gets of Johnny while you’re still connected in Pyramid Song.
Reed can’t see or hear Johnny, but he picks up from your body language that you’re talking to someone else a couple of times, though he asks if it’s Songbird communicating with you.

V yells out loud, and he explicitly tells V she doesn’t have to speak out loud, that she can think her replies so she doesn’t look like a raving lunatic in public. Johnny tells you this directly. The second time you ever speak to him. His introduction.
But to think V is able to completely hide the fact she’s talking to someone? No. Body language and all. Several characters suggest they notice V being distracted or distant, and Reed directly tells V he knows she was just talking to someone.
Like, play the game dude. jesus. pay attention. just use your brain for one second and think about how humans act. when i’m having a fake conversation with the angry voices in my head i probably look like i’m talking to someone too.

Depends how in control V is. Most of dialogues with Johnny are happening in their head. But sometimes V says things out loud.
When Johnny is making comments while V is in dialogue with other characters, you can hear the difference of how the line is said to Johnny in V’s head and how the line is said to other characters out loud.

After the beginning not rlly. At the beginning he doesn’t understand that he can just communicate through his thoughts but after he finds that out there aren’t really any times anyone can hear the two conversing.

When you meet Takemura at Tom’s Diner (while you’re sitting in the corner waiting for the waitress to pour the coffee) V yells at Johnny prompting others to look at them. Johnny explains V doesn’t need to talk out loud as he’s in their head. I’d imagine it works similarly to a private call. Whenever you get a call for a gig you can be inspecting the cyber on an enemy a few inches from them and your voice doesn’t carry
