You need to do stuff in a certain order to get this scene I believe. I think you need to do Helman’s mission before doing this one. Helman’s mission progresses the relic’s effects further and thus V becomes more like Johnny and takes up smoking automatically. Where as this scene if done first, gives you an option to refuse it.
