If you go down in a place where you can’t revive yourself, you will respawn with nothing.
You had quick revive and who’s who at the same time. Probably that whos who couldn’t find your crushed body in the elevator so quick revive took over instead
That elevator was at the top floor, so you were crushed
And as others pointed out, Who’s Who respawned you as it normally would, but because your body was not recoverable, it bled out, just like it would in a co-op game
