Happens to me all the time as well.
The actual reason is that collision in stairwells and corners is bad, much of the time half your body is hanging outside the building, there’s a good chance that someone saw your ass and gun hanging out of the edge of the building and procced high alert.
I’ve experienced this also. I figured it’s a glitch.
Your operator model was probably clipping through the wall so the the game thought you were outside of the building.

Only thing I can possibly think of would be it alerted you due to the grenade/explosion occurring on the other side of the wall. May be due to a glitch, may be due to your player partially rendering outside of the wall/house
