Not sure what causes it but something causes the game to get confused. That is why your teammates had successfully exfiled but your character was still on the chopper
Same thing happened to friend and I. We ended up miles below the map.

It reminds me of the fact that the exfil chopper in Outbreak actually just ditched you somewhere when the screen went dark.
But this is much worse.
