I’ve had similar glitches like this before. I played a few hours one time without hands, all you could see was the cyberware for the gorilla fists. It was pretty funny especially during cut scene moments
It’s a hard habit to break.

Hey !! There’s a couple fixes for this. One is going to an apartment that lets you smoke such as the japantown apartment, and also changing your clothes/outfits sometimes removes the cigarette. Make sure to save right after so it doesn’t pop back up
