It is anecdotal, but today I got sniped and I died instantly. I did have the flare too

could be the that they were using a RPK which has a higher ranged ttk coupled with the fact that since you were behind cover the AA locked on to your head, dealing even more damage.
Those are guesses, though. I actually haven’t played in a while so I could be very wrong

As per reddit user u/jamcowl, in another thread, he mentioned it was to do if you pickup the revive pistol at any moment during the game it is a bug and cause the instant death!
Since I’ve read, and start not using or picking up the revive pistol, I never has the problem!
