I got the 100% card after completing the Headhole category, but I'm only 56% done with all prestige challenges. Some guy at Raven mistakenly had it track for "any" category instead of "all" categories did they?

1: why are the “survival instinct” challenges locked? 2: my 5 melee weapon kills aren’t tracking. I’ve tried every melee weapon and they aren’t counting towards the 5… anyone else dealt with this or have a solution?
