It’s your outfit and body mods. Make sure the outfit you’re using matches the shape of your body type. Most mods should support them but some don’t.

Either a bug or a modding issue. If you don’t have mods I’d say they changing your character’s clothes a few times or relaunching the game. That worked for me when my male V’s Johnson decided to just hang out.

You need to download a refit mod, I’m not sure if this is vanilla clothing but there’s a refit mod for vanilla clothes and whatever modded clothing you installed, usually they’ll tell you if they’re compatible with the body mod you downloaded
