Effective clockspeed doesn’t always mean it’s clock stretching if it’s lower than the clockspeed you initially see. Try decreasing to -20 CO and if effective clockspeed increases then you know your answer but also keep in mind effective clockspeed isn’t always good to look at

Looks like it. You can also activate snapshot polling in the HWiNFO options, it should be a bit more accurate.
You can compare the values at different CO values, but with 100% CPU usage, that should be clock stretching.
The other way the effective clocks are lower is when the cores are going to idle states, but that would not be at 100% CPU utilization.
