I think its has to do with the fact that when Dolls are on “work” Mode, they enter in a cyber transe, same as what happens with the doll when V goes to Clouds and talks with him/her, the dolls eyes get colored and lit, meaning their body as been taken over by some algorithm, this also happened with the doll that sent Hanako Arasaka to talk in private to V.

One thing that’s easy to miss is that those are just avatars, not video of the person. So they can have it look however they want.

As others have mentioned, she chose to appear in doll work mode. It looks creepy but she has a sense of humor, and likely did that on purpose.
I love that it’s Vex’s voice actor from Skyrim, and I could totally see Vex making that decision in this context, too lol
