This happens to me sometimes as well, i think its either related to 4:3 or second monitor or both
Usually happens cause I alt-tab before the game opens properly.
Yes, that sometimes happens to me too. Mostly when the game starts and you accidentally tab out again or VERY rarely during the game. Nothing helps except killing the game via the task manager and restarting it.
This should be a known bug for Valve
