tbf when Maxtech comes they don’t say, oh aim for the head… they treat you like a Cyberpsycho, a robot more or less and yell “Aim for the center mass”.

Kinda. With the Edgerunner perk, you *can* be a cyberpsycho canonically, just a “functioning” one so to speak. Like that lady from MaxTac in that sidequest. Cyberpsycho yes, but better at her job because of it.
Think of it this way – gangsters or not, would a normal person just rock up to a group of latino guys in wifebeaters, activate military-grade reflex-enhancing cyberware, throw a grenade, and then headshot them all with a shotgun?
Depending on how you play, V can literally just leave a trail of blood through Night City. Oh, and maybe you should start thinking if this is normal after the third MaxTac EV arrives.
Though I do think that Johnny might help prevent “complete” cyberpsychosis on some level.

This is why the opening scene of Edgerunners hits home so well.

I’d imagine it has something to do with Johnny being in V’s head. Two minds taking on the same stress load. Might be staving off cyberpsychosis.
My other theory is that, if you were to become a cyberpsycho, you would basically lose your sense of self. An immediate game-over, if you will. If the player represents the conscious mind, then we would basically get disconnected from our character.
Could make for a fun gameplay mechanic though. Hallucinations, symptoms, immunosuppressants… it would add a layer to combat and character building.
The problem would be the story impact. There would have to be some kind of reaction to your character visibly demonstrating signs of cyberpsychosis.
