Probably the player character loaded into the BD “cell” (not sure the actual name or method CDPR uses for loading areas) and not being as out-of-frame as the devs likely thought they were
Either that or just an NPC that isn’t supposed to be there, or is supposed to be there but isn’t supposed to be visible/in frame
No, it is not a bugged NPC. It’s the kidnapper’s mother, you can see her ghost in other BDs of him because they are basically BDs created out of his actual dreams. You see, at the start of the mission River shows you a footage of the perpetrator getting shot by police and then he goes into a comatose state. So we start digging into his notions behind his crimes and we learn that his mother died when he was a child, caused by some sort of disease that also affected the cattle in their farm. So when he dreams, he also imagines his mother and she appears as a form of ghost in those BDs.
It is good that you noticed this easter egg, which gives the mission a bit of eeriness. It is intentional by CDPR side, so it is not a bug or glitch.
In my mind, thats her observing from the spot that shes been buried.
When you go through the recording you can also see her standing behind the plastic divider, next to the office where the computer is. When you move the divider she disappears.
