When V touched the smart glass TV and the whole screen glitches out I first thought it was my PC that croaked.
Luckily it was a very convincing in-game thing.

I had that very problem when I started to play it (on PS4 Pro) and I don’t know if this would help you, but I’ll tell you what I did and helped me (since then it doesn’t crash at all):
1.- turn off WiFi while playing
2.- modify some visual specs of the game itself like visual aberration and tracing
3.- if possible, disable auto save
Now, I’m no engineer at all but I followed these steps and my game is running smooth and perfect since then.

Had it crash during the Street Kid intro, it was during that scene where you meet Jackie and those cop beat your ass, my game crashed right when the cops club came down on V’s face to transition to the next scene, it made it look like he hit me so hard that the game broke
