Without even checking pubglookup I see you took around 25, 25 and 50 damage, so if I had to guess it was body, body, headshot. With M416 it takes 0.17 seconds to fire three shots, so literally faster than a blink of an eye.
And it was pretty much the flushing attempt that got you killed.
