I reloaded the save about 10 times doing this, every time the same.
The system most likely doesn’t take into account any perk shards you might have picked up during the game as random loot. So when you reset it it only registers the ones you got from leveling. Can’t think of anything else
Wait, how do you reset attribute points? I didn’t know that you could do that.
i had something similar such as sometimes my cyberware will go over the limit, i will remove an a piece of cyberware and try to put it back and it wont let me, somehow lose capacity.

You might be getting free perk points due to a bug with manual refunding. I’ve noticed that when you manually refund a perk, you will get the perk point back but the perk itself sometimes won’t get removed. I could then refund the same perk a second time which would successfully remove the perk but also grant an additional perk point.
In particular, I’ve noticed this with Bullet Deflect (reflex) and Acquisition Specialist (intel). I’m not sure how to reliably duplicate the bug, but it seems to only happen when other perks of the same attribute are refunded first.
