Happens to me when I equip a revive pistol (even if I drop/stow it later)

It looks like one of your teammates is already dead. Your last remaining teammate gets downed the split second right before you lose that gunfight. Since no one is left right as you lose your fight, you died instantly. Watch his health bar in your clip. It decreases rapidly as you’re fighting that last enemy.
