I wonder if it’s anything like the difficulty bugs with scripted car chase scenes. It was impossible to complete the Nomad intro on Very Hard because of the massive amount of damage you take. Jumps way over the health threshold where you’d get back in the car and you just outright die. Maybe Panam took way too much damage so that some scripted action for her to back off or go into a downed state or something was bypassed by a margin large enough that she skipped straight to dead.

I think her “no death” script has broke, I felt some of the base game quests have been a bit off after the new update.
I had an issue where I killed two nomads harassing the prophet guy, police were nearby and it somehow broke their aggro script; from there onwards they never aggro’ed (no stars either). I could cut someone in front of them and no response at all
