I ran from the Biodome to hack the excavator as soon as I heard the announcement, so it can’t be some same-frame deal. I went back to training and was shook when I heard that it was breaching.

From my experience, it seems that if an excavator breaches, is hacked, and gets activated again, it will take one less minute for the model of the excavator (but not the excavator itself) to breach. If you hack the excavator once the model has breached but the full four minutes aren’t up, then you get this glitch where the model goes back to its starting position, still in the breaching state, with an invisible excavator that breaches at the four minute mark. I believe that the excavator will remain in this broken state until it activates for a third time, at which point if you wait the full four minutes to hack it the excavator will go back to normal.
No idea if there is further glitchiness with 4+ breaches, never played a game that long
