Are you on PC? Delete Players folder under documents and see if that helps. you will have to reload and change a lot of settings back to your preference. I only have 10/10 Loadouts. Not sure how you have 12/12…?

Don’t go into the loadouts when the game textures are loading in, it will take all your attachments off your guns

I just saved my gun loadouts w/ the attachments

(At least on PC) it usually happens when you try to go into your weapon loadouts before the game fully loads the menu. Always wait at least 2-3 seconds for your match or gameplay menu to fully load before you go into your loadouts.

When your editing your class and party host changes lobby for me usually. Sometimes wipes everything.
Save your guns as blueprints for an easy fix

I had a buddy that said it was due accessing the weapons menu to quickly after loading into the main menu. It sounds dumb, and very anecdotal, but since I have been taking a pause before jumping into my loadouts I have not had the issue happen again, and it was happening to me regularly.
